Internal Groups - Facility Use Application
This form is to provide details for an ICOG Ministry event.

- To ensure the building will be closed by 10:00 PM, ALL evening events must conclude at 9:30 PM. The Ministry will be responsible for leaving the facilities used in such condition that another group may comfortably use them, i.e., furniture and equipment will be placed in the proper location, kitchenette equipment left in clean condition, floor swept and mopped and trash cans emptied and taken to the trash dumpster, etc.
- Decorations used must be flameproof and shall not be attached to fixed portions of the facilities without approval. ~ The use of candles or open flames is prohibited. The ceremonial use of candles may be permitted on occasion, with approval.
- Alcoholic beverages, liquors, or other non-physician-prescribed drugs will not be permitted on church property at any time. ~ Smoking will not be permitted on church grounds. ~ Social dancing will be permitted upon special request.
